4 month Nutrition Coaching

  • 60 minute Initial Evaluation- virtual or in-person

  • Google sheet for plan & goals; updated with each meeting

  • Weekly Google Form Check Ins

    • Assessment of action items

    • Progress measurements

  • 30 minute Check Ins, biweekly- virtual

    • Summary from coach after each meeting​

  • Communication through PtEverywhere for messaging

  • If applicable, food diary tracked through an app

can be split into 2 payments

6 month Nutrition Coaching
